Answer:
How do I claim Off Track Vehicle benefits if I am injured in a motor vehicle accident while on duty?
Off Track Vehicle benefits have risen from what used to be $150.00 per week to a current maximum of $1000.00 per week under the 2002 UTU National Agreement.

To file a claim, employees need to submit a request for OTV payments, advising all pertinent facts, i.e., date of accident, location, etc., in writing, to Debbie Fore's attention as follows:

U.S. Mail:         D. A. Fore
                        Director Labor Relations
                        CSX Transportation – J455
                        500 Water Street
                        Jacksonville, FL  32202

Email:               Debbie_Fore@csx.com

Once the request has been received, Director Fore will verify with the Risk Management group that OTV benefits are payable, and generate a check each Friday for the preceding week (payable on the regular pay day) until such time as she is advised by Risk Management that the employee has returned to service.
